• See test results quickly
• Communicate with your health-care team regarding non-urgent medical questions, referrals and test results
• You may use the Patient Portal to request medical records for personal use or referral to another provider. You may also access your medical records by filling out and submitting the Medical Record Release Authorization Form.

Do Not Use the patient portal for urgent medical problems.
