We are pleased to be your provider of cardiovascular care. We strive to provide the best possible experience; from the medical care you receive from your physician to the customer service from our support staff. Below is a list of next steps as a new patient or caregiver to help ensure your experience with AHVI is a positive one.

Schedule an Appointment

Share Your Referral and Medical Records

To prepare for your visit, your AHVI provider will review your medical records. At least one week prior to your appointment, have your primary care provider and/or referring physician fax your records to 1-800-537-4124. If you have your records, please bring them to our office prior to your appointment.

Failure to have medical records or referrals available may result in difficulty in providing quality care.

Telehealth Updates

Telehealth Flexibilities End September 30, 2025
In March 2020, the Department of Health and Human Services (HHS) waived many restrictions on telehealth in response to the COVID-19 public health emergency, allowing patients to receive care from home using a smartphone, computer, or tablet. This temporary expansion made healthcare more accessible, convenient, and affordable for many people and was extended several times through legislation.
The final extension expires on September 30, 2025, and without further congressional action, the regulations governing telehealth will return on October 1, 2025, to the more restrictive standards that were in place before the pandemic.
 
What This Means for Patients
Starting October 1, 2025, appointments with Alaska Heart & Vascular Institute providers must take place in person at one of our clinic locations in Anchorage, Wasilla, or Soldotna. Current federal law does not allow patients to receive care from home through telehealth, even for those who live in rural areas or have difficulty traveling.
 
We recognize that this change may create challenges, especially for patients who live in rural areas. Our team is here to help coordinate your care, schedule appointments, and answer any questions you may have. If you have an upcoming appointment scheduled after October 1, 2025, please plan to attend in person. Our staff will work with you to ensure you are scheduled at the location most convenient for you.

If you believe all patients should continue to have access to telehealth services, we encourage you to contact your congressional representatives and urge them to reinstate telehealth flexibilities. You can make your voice heard by sending a message to your representatives using this link.

Self Check-in

If you have opted in to receive email and/or text alerts to remind you of your appointment, you will be given the opportunity to Self Check-in up to 48 hours in advance of your appointment. By clicking the link in this email or text, you can check-in early for your appointment and make sure all necessary paperwork and information is up to date!

Please note that even if you complete the Self Check-in process, we will need you to physically check-in at the front desk when you arrive for your appointment, so our staff knows you have arrived.

Attending Your Appointment

Please be prepared to bring the following items/information to your appointment:

  • Current drivers License or other valid government-issued photo ID
  • Health insurance card
  • Payment for co-pay, deductible, or co-insurance applicable to your health plan
  • Prescription medication bottles, as well as a list of any vitamins and supplements you take
